Complete the form below to book your telemedicine appointment (card #8 of the form), submit your medical intake, and pay the $50 evaluation fee — all in one step. Appointments are available Monday through Sunday, 12 PM to 5 PM, by phone.
Certification is based on an individual clinical evaluation and is not guaranteed. See our Cancellation & Refund Policy — the evaluation fee is non-refundable.
